Get the Texas Application for Persons with Disabilities Parking Placard and/or License Plate (Form VTR-214)
Have a licensed physician, podiatrist, chiropractor, optometrist, or advanced practice registered nurse/physician assistant complete the medical certification section, if required
Make sure you qualify with a disability that limits walking or meets Texas eligibility rules
Submit the completed application to your local county tax assessor-collector office
Bring valid identification and any required vehicle information if applying for plates
Pay any required fees for license plates; permanent placards are typically free
Request a temporary placard if your condition is temporary and expected to improve
Renew the placard before it expires if you still qualify
Keep the placard visible when parking in accessible spaces
Do not use the placard unless the person it was issued to is being transported or is using the parking space
