Complete Form DS-11 for a first-time passport, minor passport, or if your previous passport was lost, stolen, or expired more than 15 years ago
Gather proof of U.S. citizenship, such as a certified birth certificate or naturalization certificate
Gather a valid government-issued photo ID, such as a driver’s license or state ID
Bring a photocopy of the front and back of your ID
Get one passport photo that meets U.S. passport requirements
Apply in person at a passport acceptance facility in Ohio, such as a post office, clerk of court, public library, or county office
Pay the required fees for the passport application and execution fee
