Download the Chapter 7 forms packet (often labeled “Voluntary Petition for Individuals Filing for Bankruptcy” and related schedules)
Complete these core forms:
Voluntary Petition for Individuals Filing for Bankruptcy (Form 101)
Schedules A/B, C, D, E/F, G, H, I, J
Statement of Financial Affairs (Form 107)
Means Test documentation (Form 122A-1 or 122A-2, as applicable)
Declaration About an Individual Debtor’s Schedules (Form 106Dec)
List of Creditors (usually includes mailing list requirements)
Confirm you have completed required pre-filing credit counseling (certificate must be filed with the petition)
Complete the required debtor education course after filing (certificate filed later, per court deadline)
File electronically through the court’s e-filing system (or file paper if e-filing is not available/required)
Pay the filing fee or request a fee waiver/installment plan if eligible
Submit all completed forms, creditor mailing list, and required certificates with the petition
Review the docket for filing confirmation and assigned case number
Serve notice to creditors according to the court’s instructions (if required)
Track deadlines for meeting of creditors (341 meeting) and any required amendments
