Create or sign in to your my Social Security account at ssa.gov/myaccount
Check whether you are eligible to request a replacement card online
If eligible, submit the replacement card request through your account
If not eligible online, complete Form SS-5, Application for a Social Security Card
Gather required documents proving identity and, if needed, citizenship or immigration status
Submit the application and documents by mail or in person to your local Social Security office
Use the Social Security Office Locator at ssa.gov/locator to find the nearest office
Wait for the replacement card to arrive by mail
