Open the Health app
Tap your profile picture in the top-right corner
Tap Medical ID
Tap Edit
Tap Add Emergency Contact
Select a contact
Choose the relationship
Tap Done
Tap Done again to save
To view Emergency Contacts from the lock screen, press and hold the Side button and Volume button, then tap Emergency ID or Medical ID
