Determine whether you are eligible for a U.S. passport
Complete Form DS-11 if applying for the first time, under age 16, or if your previous passport was lost, stolen, or damaged
Do not sign Form DS-11 until instructed to do so by the acceptance agent
Gather proof of U.S. citizenship
Gather a valid government-issued photo ID
Bring a photocopy of the front and back of your ID
Obtain a passport photo that meets U.S. passport requirements
Calculate and prepare the required fees
Apply in person at a passport acceptance facility or passport agency
Submit your application, documents, photo, and payment
Provide parental consent and additional documents if the applicant is under 16
Track your application status after submission
Receive your passport by mail once approved
