Open the Phone app
Dial `##21#` and press call
Dial `##002#` and press call
Dial `##61#` and press call
Dial `##62#` and press call
Dial `##67#` and press call
Go to Phone settings
Tap Call settings
Tap Call forwarding
Turn off all forwarding options
Contact your mobile carrier if forwarding still remains
