Visit your home club in person and request a cancellation form
Ask a club representative to process the cancellation and provide a confirmation number or receipt
Send a written cancellation request by certified mail or email if your agreement allows it
Include your full name, membership number, phone number, email, and home club location in the request
Keep copies of all documents, emails, receipts, and confirmation details
Follow up with the club or member services to verify the cancellation was completed
Check your bank or credit card statements to confirm billing has stopped
If required by your contract, return any access card or key fob to the club
