Carry a valid proof of identity showing the correct surname
Fill out the Aadhaar Update/Correction Form
Select the surname correction option
Enter the correct surname exactly as per the supporting document
Submit the form along with the original document for verification
Provide biometric verification if asked
Pay the applicable update fee
Collect the acknowledgement slip with the Update Request Number
Check the update status online using the URN
Download the updated Aadhaar after the surname change is approved
