How To Create A Form In Excel?

Open Excel and create or open a workbook

Go to File

Click Options

Select Customize Ribbon

Check the Developer box

Click OK

Go to the Developer tab

Click Insert

Choose a form control such as Text Box, Check Box, Option Button, or Combo Box

Click and drag on the worksheet to place the control

Right-click the control and select Format Control if needed

Link the control to a cell if required

Enter labels and field names in adjacent cells

Format the worksheet as needed

Save the workbook
