Be a member of the U.S. Armed Forces

The injury must be caused by enemy action
The injury must require treatment by a medical officer
The treatment must be officially documented
The wound must have occurred while serving under competent authority
Submit supporting records through the chain of command
Have the award approved by the appropriate military authority
