Gather required documents proving identity, age, and U.S. citizenship or lawful immigration status
Complete Form SS-5, Application for a Social Security Card
Submit the application and original documents to your local Social Security office or by mail if allowed
If applying for a child, provide the child’s birth certificate and proof of the parent’s identity
If you are a noncitizen, provide immigration documents such as a work permit, visa, or permanent resident card
Wait for the Social Security Administration to process your application
Receive your Social Security card by mail
Replace a lost or stolen card by completing Form SS-5 again and providing the required documents
Update your card if your name changes by submitting legal name-change documents and Form SS-5
