Complete the Social Security card application (Form SS-5)
Choose your application method:
Online (if you qualify)
In person at your local Social Security office
By mail through your local Social Security office
If applying by mail, make copies of documents and follow the office’s submission instructions
Submit the application and required documents
Receive your Social Security card by mail (timing varies)
If you need a replacement card, indicate “replacement” on Form SS-5 and provide any required supporting documentation
If you need the card for a child, apply for the child’s card using Form SS-5 with the child’s documents and the parent/guardian’s identity documents
If you have a name change, bring documentation of the legal name change and indicate the change on Form SS-5
