Check Box 5 for Medicare wages and tips

Review Boxes 7 and 8 for Social Security tips and allocated tips if applicable

Check Box 10 for dependent care benefits if applicable

Review Box 12 for codes and amounts such as retirement contributions or health coverage

Check Box 13 for retirement plan, statutory employee, or third-party sick pay indicators

Review Box 14 for other deductions or information from your employer

Check Box 15 for your state and employer state ID number

Review Box 16 for state wages, tips, and other compensation

Check Box 17 for state income tax withheld

Check Box 18 for local wages, tips, and other compensation if applicable

Check Box 19 for local income tax withheld if applicable

Check Box 20 for the locality name if applicable

Compare the form with your final pay stub and year-end records
