Stop all potassium intake
Discontinue potassium-raising medications
Place the patient on cardiac monitoring
Obtain an ECG
Give intravenous calcium gluconate for ECG changes or severe hyperkalemia
Give insulin with dextrose
Give nebulized albuterol
Give sodium bicarbonate if metabolic acidosis is present
Remove potassium with loop diuretics if appropriate
Remove potassium with potassium binders
Remove potassium with sodium zirconium cyclosilicate or patiromer if appropriate
Use hemodialysis for severe, refractory, or renal failure–associated hyperkalemia
Recheck serum potassium frequently
Treat the underlying cause
