Provide the child’s name, date of birth, place of birth, parents’ names, and hospital details
Submit supporting documents such as hospital birth record, parents’ identity proof, and address proof
Pay the prescribed fee, if applicable
Collect the birth certificate from the local body office or download it online if issued digitally
If the certificate is lost, apply for a duplicate copy at the same office or through the online portal
If there is any error in the certificate, request correction with supporting documents at the issuing authority
For births in hospitals, ask the hospital to send the birth report to the registrar for timely registration
