Reach the blood swamp area
Defeat the Nameless White Mask invaders
One of them drops the White Mask
If you kill Mohg first, the White Mask invaders will no longer spawn
Get it before defeating Mohg, Lord of Blood
If you already killed Mohg, the mask cannot be obtained in that playthrough
