Complete Form SS-5, Application for a Social Security Card
Gather required original documents or certified copies proving identity, age, and U.S. citizenship or lawful immigration status
If applying for a child, gather the child’s birth certificate and proof of the parent’s identity
Visit your local Social Security office or mail your application and documents if allowed
Submit the completed Form SS-5 with all required documents
Wait for the Social Security Administration to process your application and mail your card
If you are replacing a lost or stolen card, request a replacement using the same application process
If you are changing your name or correcting information, provide legal documents supporting the change
