Open the Health app

Tap your profile picture in the top-right corner

Tap Medical ID

Tap Edit

Scroll to Emergency Contacts

Tap Add Emergency Contact

Select a contact

Choose the relationship

Tap Done

Tap Done again to save the Medical ID
