Check whether you are eligible to request a replacement card online
Gather required documents, if needed, such as proof of identity and U.S. citizenship or immigration status
Complete the Social Security card application online or download Form SS-5
Submit the application online if eligible, or mail or take the completed form and documents to your local Social Security office
Wait for the replacement card to arrive by mail
