Sign in to your Google account
Open Google Forms at forms.google.com
Click Blank form or choose a template
Enter a title for your survey
Add a description if needed
Click the question field to type your first question
Choose the question type from the dropdown
Add answer options if required
Toggle Required if the question must be answered
Click the plus icon to add more questions
Use the theme palette to customize the form
Open Settings to adjust response options
Enable or disable email collection if needed
Set response limits or quiz options if needed
Click Preview to review the survey
Click Send to share the form
Copy the link or send by email
Check responses in the Responses tab
