Go to Google Forms at forms.google.com
Sign in to your Google account
Click Blank form or choose a template
Enter a title for your survey
Add a description if needed
Click the plus button to add a question
Choose the question type
Type your question and answer choices
Mark questions as required if needed
Add more questions as needed
Use the theme button to customize the design
Preview the survey using the eye icon
Click Settings to adjust form options
Click Send to share the survey
Copy the link, email it, or embed it on a website
Review responses in the Responses tab
