Relieve pressure from the sore immediately
Reposition the person at least every 2 hours
Use pillows or cushions to offload bony areas
Keep the skin clean and dry
Clean the sore gently with saline or mild soap and water
Remove dead tissue only by a healthcare professional
Apply dressings as directed by a clinician
Use pressure-relieving mattresses or overlays
Maintain good nutrition with enough protein, calories, vitamins, and fluids
Control moisture from sweat, urine, or stool
Treat pain as prescribed
Watch for signs of infection such as redness, warmth, swelling, pus, or fever
Seek medical care for deep, large, worsening, or infected sores
Follow wound care instructions from a doctor or wound specialist
