Log in to the Marketplace website and select tax forms or documents
Review your email or mailed notices from the Marketplace for availability of the form
Contact the Marketplace Call Center if you cannot access the form online
Request a corrected Form 1095-A if the information on the form is wrong
Keep the form for filing your federal tax return if you had Marketplace coverage
Use Form 1095-A to complete IRS Form 8962 if you claim the premium tax credit
