Request a 504 evaluation from the school in writing
Send the request to the school principal, counselor, or 504 coordinator
Keep a copy of the request for your records
Meet with the school’s 504 team for evaluation
Share relevant medical and academic information with the team
Ask for specific accommodations that address the student’s needs
Review the school’s decision after the evaluation
If approved, participate in creating the 504 plan
Make sure the plan lists clear accommodations and supports
Ask how the plan will be shared with teachers and staff
Monitor how the plan is being implemented
Request a review if the plan is not working
Revisit the plan as the student’s needs change
If denied, ask for the reason in writing
Consider appealing the decision through the school district’s process
