Complete the DS-11 form (Passport Card application)

Submit a color photo that meets U.S. passport photo requirements
Get your application submitted in person at an acceptance facility (post office, clerk of court, or other authorized location)
Pay the required Passport Card fees (and any acceptance facility fees)
If required, bring supporting documents (e.g., custody documents for minors, name-change documentation)
Receive your passport card by mail after processing
If you already have a valid U.S. passport book and are eligible to apply by mail, submit the required forms and documents using the applicable process
