Fill out Form DS-11 if applying for your first U.S. passport, a child passport, or a replacement after loss/theft
Gather proof of U.S. citizenship
Gather a valid government-issued photo ID
Bring a photocopy of the front and back of your ID
Get a passport photo that meets U.S. passport requirements
If applying for a child, bring the child and both parents or required consent documents
Find a passport acceptance facility in New Jersey such as a post office, county clerk, or municipal office
Make an appointment if the acceptance facility requires one
Bring completed forms, documents, photo, and payment to the acceptance facility
Pay the passport application fee and acceptance fee if required
Track your application after submission
Request expedited service if you need the passport sooner
Renew by mail with Form DS-82 if you qualify instead of applying in person
