Complete Form DS-11 if applying for your first passport, your previous passport was lost or stolen, or your previous passport was issued when you were under 16
Gather proof of U.S. citizenship, such as a certified birth certificate or naturalization certificate
Gather a valid government-issued photo ID, such as a driver’s license or state ID
Bring a photocopy of the front and back of your ID
Obtain a passport photo that meets U.S. passport requirements
Find a passport acceptance facility in New York, such as a post office, clerk’s office, or library
Make an appointment if the acceptance facility requires one
Bring the completed DS-11 form but do not sign it until instructed
Bring all original documents and copies to the appointment
Pay the passport application fee and the execution fee if applying in person
Submit the application in person at the acceptance facility
Track your application status online after submission
Request expedited service if you need the passport faster
Apply for a passport book, passport card, or both, depending on your travel needs
Renew by mail using Form DS-82 if you qualify for renewal
Replace a lost or stolen passport using Form DS-11 and Form DS-64 if applicable
