Complete the Social Security Number (SSN) application on Form SS-5
Provide the baby’s certified birth certificate or hospital birth record
Provide proof of the parent’s identity, such as a driver’s license or passport
Provide proof of the parent’s relationship to the baby, if requested
Apply through the hospital if it offers the newborn SSN application service
Apply at a local Social Security office if not done through the hospital
Apply as soon as possible after the baby is born
Wait for the Social Security card to arrive by mail
