How to Get an Autopsy Report?
Contact the county medical examiner or coroner’s office that handled the case
Contact the hospital, if the death occurred in a hospital and an autopsy was performed there
Identify the decedent, date of death, and case number if available
Ask whether the autopsy report is complete and available for release
Submit a formal records request in writing if required
Provide proof of your identity
Provide proof of relationship or legal authority if you are next of kin, executor, or attorney
Complete any required authorization or release forms
Pay any required copy or processing fees
Ask about any waiting period before the report can be released
Request the report by mail, email, fax, or in person if those options are offered
Follow up with the office if you do not receive a response
If the report is sealed or restricted, ask what legal process is needed to obtain it
If needed, request the death certificate separately from the vital records office
