Open the PDF in Adobe Acrobat Pro
Go to Tools
Select Prepare Form
Choose the PDF file if prompted
Let Acrobat detect form fields automatically
Review the detected fields
Add fields manually if needed
Use the toolbar to insert text fields, checkboxes, radio buttons, dropdowns, or signature fields
Resize and position the fields as needed
Double-click each field to set properties
Save the fillable PDF
Test the form by opening it and entering data into the fields
