Create or obtain the PDF you want to make fillable
Open the PDF in Adobe Acrobat Pro
Click Tools
Select Prepare Form
Choose Select a file and upload/open the PDF
Acrobat will detect fields automatically; review and adjust as needed
Add fields using the form tools: Text Field, Check Box, Radio Button, Drop-Down List, Signature, Date, or Initials
Draw or place each field on the correct area of the PDF
Double-click each field to set properties (name, required, formatting, validation, default value)
Set field appearance and alignment so text fits properly
Test field behavior by clicking into each field and entering sample data
Link fields across pages if needed (same field name/logic)
Save the updated PDF as a fillable form
Click Preview to verify the form works as intended
Distribute the form and ensure users can fill and save responses
If needed, enable saving: ensure users can fill and save the filled PDF (use Acrobat save behavior settings)
