Visit the official PM-JAY website or the nearest Ayushman Bharat center
Carry Aadhaar card, ration card, mobile number, and any other required identity proof
Search your name in the PM-JAY beneficiary list
If your name is listed, go to an empaneled hospital, CSC, or Ayushman kiosk
Complete e-KYC using Aadhaar or other approved documents
Provide family details and verify your eligibility
Get your Ayushman card printed or downloaded after approval
If your name is not listed, visit the nearest CSC or district Ayushman help desk for verification
Use the official PM-JAY app or website to check card status and download the card after it is generated
