Complete Form DS-11 for a first-time passport, lost/stolen passport, or if you do not qualify to renew by mail
Gather proof of U.S. citizenship, such as a certified birth certificate, naturalization certificate, or previous U.S. passport
Gather a valid government-issued photo ID, such as a driver’s license or state ID
Obtain one passport photo that meets U.S. passport photo requirements
Make photocopies of your citizenship document and ID
Find a passport acceptance facility, such as a post office, clerk of court, or library
Submit your application in person if required
Pay the required passport fees
Provide any additional documents requested by the acceptance agent
Track your application status after submission
Receive your passport by mail when approved
