Have a licensed Florida physician, physician assistant, advanced practice registered nurse, podiatrist, or optometrist certify your disability on the Florida application
Complete Form HSMV 83039, Application for Disabled Person Parking Permit
Submit the completed application to your county tax collector’s office or license plate agency
If applying for a temporary permit, request the appropriate temporary parking permit duration
If approved, receive your disabled parking placard or license plate
Renew the permit before it expires if it is temporary or requires renewal
