Complete Form DS-11 if applying for a first-time passport, a child passport, or a replacement for a lost/stolen/damaged passport
Use Form DS-82 if you qualify for passport renewal by mail
Gather proof of U.S. citizenship, such as a certified birth certificate or naturalization certificate
Gather a valid government-issued photo ID
Bring a photocopy of the front and back of your ID
Obtain one passport photo that meets U.S. passport requirements
Check the current passport fee schedule
Find a passport acceptance facility in New York, such as a post office, clerk’s office, or library
Make an appointment if required by the acceptance facility
Submit your application in person if using Form DS-11
Sign the application in front of the acceptance agent
Pay the required fees
Track your application status after submission
Receive your passport by mail when approved
