Identify the branch of service and the type of records needed

Gather the veteran’s full name, date of birth, Social Security number, service number if available, branch, and dates of service

Determine whether the records are service treatment records, personnel records, or VA medical records

For service treatment and personnel records, submit a request to the National Personnel Records Center

Use Standard Form 180 to request military records

Complete all required sections of Standard Form 180 accurately

Sign and date the request

Include proof of identity if required

Submit the request by mail, fax, or through the appropriate online records request system if available

If requesting records for someone else, include authorization or proof of legal authority

If the veteran is deceased, provide proof of death and proof of relationship or legal authority if needed

If requesting VA medical records, contact the VA medical center where treatment was received

If requesting current military treatment records, contact the military treatment facility or records office
