Go to Falion in Morthal
Ask him about curing vampirism
Buy a filled black soul gem
Fill the black soul gem if needed
Return to Falion at dawn
Meet him at the summoning circle outside Morthal
Let him perform the cure ritual
If you are a vampire lord, cure that first if needed
If you are in the Dawnguard questline, complete the cure through Falion the same way
