Gather proof of your legal name change, such as a marriage certificate, divorce decree, or court order
Gather proof of identity, such as a valid driver’s license, state ID, or U.S. passport
Gather proof of U.S. citizenship or lawful immigration status if required
Submit the completed application and original or certified documents to your local Social Security office or by mail if allowed
Wait for the Social Security Administration to process your request
Receive your updated Social Security card with your new name
Update your name with the DMV, employer, bank, and other agencies after Social Security records are changed
