Check your membership agreement for cancellation terms and required notice
Visit your home club in person and request a cancellation form
Fill out and sign the cancellation form
Ask for a copy of the completed form or written confirmation
If allowed by your club, send a cancellation letter by certified mail to your home club
Include your full name, membership number, address, phone number, and request to cancel
Keep copies of all documents and proof of delivery
Cancel any recurring bank payments only after confirming the membership is canceled
Follow up with the club to verify the cancellation is complete
Return any club-issued access items if required by your location
