Open Microsoft Word
Go to File > Options > Customize Ribbon
Check the Developer box and click OK
Place your cursor where you want a form field
Use the Developer tab to insert a content control
Choose Plain Text Content Control for text input
Choose Rich Text Content Control for formatted text
Choose Drop-Down List Content Control for selectable options
Choose Combo Box Content Control for editable options
Choose Date Picker Content Control for dates
Choose Check Box Content Control for checkboxes
Select a content control and click Properties to set its title, tag, or options
Add all required form fields
Use Developer > Design Mode to adjust the form layout if needed
Protect the form by going to Developer > Restrict Editing
Select Filling in forms under editing restrictions
Click Yes, Start Enforcing Protection
Save the document as a Word template or regular Word file
