Open the Instagram app

Tap Accounts Center
Tap Accounts
Select the Facebook account
Tap Remove from Accounts Center
Confirm the removal
If needed, open Facebook
Go to Settings and privacy
Tap Accounts Center
Tap Accounts
Select the Instagram account
Tap Remove from Accounts Center
Confirm the removal
