Look for a mailed copy from the Marketplace
Contact the Marketplace Call Center if you cannot find it
Check with the primary account holder if someone else enrolled you
Verify your mailing address is current with the Marketplace
Use Form 1095-A from the prior tax year if needed for tax filing
Request a replacement copy from the Marketplace if necessary
