Complete Form DS-11 for a first-time passport, child passport, or if your previous passport was lost, stolen, or expired too long ago
Gather proof of U.S. citizenship, such as a certified birth certificate, previous passport, or naturalization certificate
Gather a valid government-issued photo ID, such as a driver’s license or state ID
Bring a photocopy of the front and back of your ID
Get one passport photo that meets U.S. passport requirements
Find a passport acceptance facility in Indiana, such as a post office, clerk’s office, or public library
Make an appointment if the facility requires one
Bring all documents, the completed application, and payment to the acceptance facility
Pay the passport application fee and execution fee
If applying for a child under 16, both parents or guardians should appear unless special rules apply
Wait for processing after submitting your application
Check your application status online if needed
Receive your passport by mail when approved
