Complete Form DS-11 if applying for your first passport, a child passport, or replacing a lost/stolen/damaged passport
Gather proof of U.S. citizenship
Gather a valid government-issued photo ID

Obtain a passport photo that meets U.S. passport requirements
If applying for a child, bring the child and the required parental consent documents
Find a passport acceptance facility in Massachusetts
Make an appointment if required by the acceptance facility
Submit your application in person at the acceptance facility
Pay the application fee and execution fee
Track your passport application status online after submission
Wait for processing and receive your passport by mail
Use Form DS-82 by mail if renewing an eligible passport
Use Form DS-11 and apply in person if your renewal is not eligible by mail
