Complete Form DS-11 if applying for your first passport, if your previous passport was issued when you were under 16, if your previous passport was lost or stolen, or if your previous passport was issued more than 15 years ago
Gather proof of U.S. citizenship, such as a certified birth certificate, previous U.S. passport, naturalization certificate, or certificate of citizenship
Gather a valid government-issued photo ID, such as a driver’s license, state ID, or military ID
Bring a photocopy of the front and back of your ID and citizenship document
Get a passport photo that meets U.S. passport requirements
Find a passport acceptance facility in Pennsylvania, such as a post office, county clerk, library, or courthouse
Make an appointment if the acceptance facility requires one
Bring all documents, the completed application, the photo, and payment to the acceptance facility
Pay the application fee and the execution fee separately if required
Sign the application in front of the acceptance agent
Track your application status after submission
Request expedited service if you need your passport sooner
Use Form DS-82 by mail if you are eligible to renew your passport by mail
Use Form DS-64 if your passport was lost or stolen
Apply for a passport card if you need a land or sea travel document instead of a passport book
