Fill out Form SS-5, Application for a Social Security Card
Provide the baby’s birth certificate or other proof of birth
Provide proof of the parent’s identity
Provide proof of the parent’s relationship to the baby
If applying at the hospital after birth, complete the Social Security section on the birth registration form
If not applying at the hospital, submit the application to your local Social Security office or by mail
Wait for the Social Security card to arrive by mail
Contact the Social Security Administration if you need help or have not received the card after several weeks
