Create or sign in to your my Social Security account
Check whether you are eligible to request a replacement card online
If eligible, submit the online application for a replacement Social Security card
If not eligible online, complete Form SS-5
Gather required documents, such as proof of identity and U.S. citizenship or immigration status if needed
Take or mail the completed Form SS-5 and documents to your local Social Security office
Visit a local Social Security office in person if required
Wait for the replacement card to arrive by mail
