How To Get DD214?

Request it from the National Personnel Records Center (NPRC) using Standard Form 180

Submit the request online through the National Archives veterans records request portal

Contact your branch of service personnel records office

Check with your county or state veterans affairs office for help requesting it

If you are retired or separated recently, contact your former military unit or installation records office

If you need a replacement, request a certified copy from the National Archives

If you are the next of kin, provide proof of relationship and the veteran’s identifying information

Include full name, service number or SSN, branch, dates of service, and date of birth

Provide a signed authorization if requesting on behalf of the veteran

If the record was lost in the 1973 fire, submit alternate information and any supporting documents
