Provide the deceased person’s date of birth
Provide the deceased person’s date of death
Provide your name and contact information
Ask the funeral home whether they will report the death to Social Security
Return any Social Security payments received after the date of death
Do not cash or spend any payments received after death
Report the death as soon as possible
